In creating my online Driver Profile, I understand that this information, which is shown in my Driver Profile, will be included when I respond to Driver Wanted postings, or when I post a Driver Available Ad. I give my permission for companies viewing my ads and driver profile to order Motor Vehicle Records, Driver Accident Reports, and perform experience and background checks.

I certify that I have personally completed this profile and that all of the information is correct and true.

Release & Permission:
I hereby request and give my permission and authorization for any companies reviewing my profile to investigate at any time my background, for driving opportunities purposes, which may include, but is not limited to, any information relating to my driving records, character, general reputation, criminal history, past experience and other background checks, alcohol or drug test results, or any information about me which may reflect upon my driving credientials gathered from any individual, organization, agency, or other source, which may have knowledge about any such items of information.

Availability of Information: 
I certify that I have completed this profile of my own free will and hold Courierboard Driver.com, Courierboard LLC, and its affiliates, harmless for all liability for providing this profile and profile for my use in seeking driving opportunities through CourierboardDriver.com. I am aware that when I post a Driver Available Ad or apply for a driving opportunity on Courierboarddriver.com, the information in my driver profile will be attached and can be viewed by parties who are registered members of Courierboard.com and search for driver postings on that website. 

I understand that this could result in my receiving unsolicited messages from these parties for which Courierboard.com and Courierboarddriver.com is not responsible.  I understand that all information I have included in my Driver Available Ads and my Driver Profile is voluntarily provided by me, and it is with my full consent that this information is made available to registered members of Courierboard.com.  I understand I have the option to not answer questions when setting up my Driver Profile, and may opt not to provide information.  I understand I may make changes or updates to information in my Driver Available Ads or Driver Profile, at my discretion.
